Any registrations after July 20th will be considered late.

Registration rates

Early registration –   Flag Football & Cheer (5-6) $50 includes raffle tickets

Tackle Football & Cheer (7-15) $75 includes raffle tickets

Regular registration –Flag Football & Cheer (5-6) $60 includes $40 in raffle tickets

Tackle Football & Cheer (7-15) $100 includes $50 in raffle tickets

Late registration –      Flag Football & Cheer (5-6) $75 no raffle tickets

Tackle Football & Cheer (7-15) $125 no raffle tickets

Required at registration:
1.    Birth Certificate
2.    Contract filled out (available at registration)
3.    Physical dated on or after January 1, 2011 on  the pop Warner form located at http://www.popwarner.com/admin/forms.asp (must be signed by the
Physician with address & phone number not a R.N. or an L.P.N.)
4.    Picture
5.    Report Card with all 4 quarters (must have both sides of the report card
Including school name & students name)
6.    Proof of address – Boundary map will be at registration
7.    Parents Code of Ethics (available at registration)
8.    Player must be at registration – football players must be weighed & cheerleaders will need to be fitted for apparel
(Cheerleaders will be required to purchase separate apparel the costs will be available at registration)

Must be 5 or older on or before July 31, 2011

Divisions will be determined by age & weight (football only)

Information will be available at registration.

Bring a new friend (never played or cheered before). Receive $10 off registration.
10% discount will be applied for those that sold 15 boxes or more of cookies last year.

Boundary Area for Eastwood

You MUST live in the designated area to play for Eastwood.
All of 13206, 13203 and parts of 13208.
